Community Support Worker
Description	Case Manager for adult clients with serious and persistent mental illness. This is a community based job opportunity.
Responsibilities	Provide community based case management, linking, coordination services for a caseload of adults with severe and persistent mental illness. Work as part of a team to provide comprehensive quality care to mentally ill adults in Independence and surrounding areas. Transportation of clients to various appointments is part of the job responsibilities.
Qualifications	Bachelor's degree in social work, psychology or a related field. Preference given to those who have had experience working with SPMI population though this is not necessary to apply.
