
CHILD YOUTH MASTERS COMMUNITY SUPPORT SPECIALIST
Description
In accordance with agency Mission, Vision, Values and Goals, Youth Community Support Specialist assists eligible children/youth and their families in gaining access to needed psychiatric and psychological treatment, medical, social, educational, vocational and other supports essential to maximizing the child/youth’s adjustment and functioning within the family setting and the community. The Masters level CSS arranges, coordinates and completes assessments, assists child/youth/family in formulation of individualized treatment plan, coordinates service/treatment plan implementation, monitors service plan delivery, and documents all aspects of Community Support services as they relate to the client/family.
· Provides home and community-based support and case management services for children/youth who meet the Department of Mental Health criteria for severe emotional disturbance (SED) and/or Community Psychiatric Rehabilitation and their families.
· Arranges and completes, with the treatment
team, assessments needed for assigned
clients/families.
· Coordinates and monitors service delivery and
support activities on behalf of assigned
clients/families.
· Acts as liaison to the community, service organizations, schools and other programs on behalf of assigned clients/families.
· Assists consumer/family with treatment planning/reviews for assigned clients/families in clinically appropriate and timely fashion.
· Provides crisis intervention to assigned clients, responding to crisis situations during the week, and by serving on rotation during weekends and holidays, as member of the program’s crisis response system for all team clients.
· Participates in and meets ethical/agency standards regarding all treatment and documentation practices, QI/QA procedures, Utilization Management, emergency procedures, and program/client outcomes.
· Completes and submits SAL’s and
accompanying progress notes to supervisor
on a daily basis.
· Follows all agency policies and procedures
including emergency procedures.
Requirements
· A minimum of Masters degree in psychology, social work, counseling, nursing or closely related human services field.
· Flexibility in scheduling work week to include
evenings and weekends as needed by
clients/families.
· Availability, via pager, for crisis intervention/emergencies 24 hours a day Monday through Friday for assigned clients. Rotate emergency on-call weekends and holidays with team members.
· One year of experience (required if degree is in nursing), working with children/youth with severe emotional disturbance and their families.
· Good oral and written communication skills.
· Must have access to a dependable automobile, with adequate auto insurance, to make regular home/community-based visits; willingness to transport clients/families as needed
